Certificates of Training


State Certificates

Youth successfully completing the requirements of the program shall receive the state certificate. This certificate is valid for four years from date of issuance. Example certificate sheet can be found on pages 5-6 and 5-7.

Items included on the certificate to be completed by the certifying authority or instructors are:

  1. Certificate Number

    A numbering system has been established for the certificates. The numbers will reflect the county and date issued.

    An example number is AA-95-001-W.

    AA — Each county has been assigned a two letter county code. See page 5-8 for a listing of county codes.

    95 — The next two numbers shall be the year identification. The last two digits of the year the certificate is issued will be used.

    OO1 — Each county should start with number 001 and ascend the numbers to complete all participant certificates.

    W — The "W" will be used to indicate that this is the

    Wisconsin Certificate in the data base.

  2. Date of Issuance - day on which certificates where issued. The certificates will be good for four years from this date.
  3. Social Security Number - will be used for identification purposes only.
  4. Participant's Name
  5. Date of Birth Information should be in the order of month, day and year.
  6. Sex Indicate male with an "M" or female with an "F".
  7. Address Address should include street, P.O. box or rural route number plus the city or town of home residence.
  8. Zip Code Please use the accurate zip code for the city or town of residence.
  9. Certifying Authority The extension faculty or authorized agricultural education instructor responsible for the program. This signature represents that the program has met requirements set forth by state guidelines. Certificates are not valid without this signature.
  10. Instructor The signature of the individual providing instruction and coordination of testing procedures. It represents that the youth has successfully completed all requirements of the program.

Youth age 14 or older may also receive the U. S. Department of Labor Certificate of Training upon successfully completing the program. The federal certificate can not be issued unless the youth is 14 years of age or older. See pages 5-1,5-2,and 5-3 for federal certificates of training requirements.
 

Recordkeeping

The certificates are printed on durable paper and can be folded to make them easier for the youth to carry with them when operating on a public road. Since there are no duplicate copies a participant registry will be used for recordkeeping purposes by the certifying authority and the Wisconsin Center for Agricultural Safety and Health. It would be recommended that programs keep a photocopy of the certificates on file along with the participant registry.
