Certificates of Training


Federal Certificates

Youth successfully completing the requirements of the Wisconsin Safe Operation of Tractor and Machinery Certification program and who are 14 years of age or older are eligible to receive the federal certificate of training. A sample certificate can be found on page 5-3.

Items included on the certificate to be completed by the certifying authority or instructor are:

  1. Certificate Number
    A numbering system has been established for the certificates. The numbers will identify county and year issued.

    An example number is USDL-AA-95-001.

    USDL — is preprinted on the federal certificate.

    AA — Each county has been assigned a two letter code. See the listing of county codes.

    95 — The next two numbers shal be the year that the youth attended training and received the state certificate. The last two digits of the year of training will be used.

    — Each county should start with the number 001 and ascend the numbers to complete all participant certificates.

  2. Participant's Name
  3. Date of Birth
    Information should be in order of the month, day and year.
  4. Date
    This is the date that the certificate is issued to the youth. Youth should show their state certificates of training or verified by county records that the state certificate was received.
  5. Address
    Address should include street, P.O. box or rural route number plus the city or town of home residence.
  6. Zip Code
    Please use the accurate zip code for the city or town of residence.
  7. 4-H or Vo-Ag
    If the program is conducted by UWEX or is a cooperative program with agricultural education instructors, please indicate 4-H. If the program is conducted solely by an agricultural education instructor, please indicate Vo-Ag.
  8. Tractor Operation or Tractor and Machinery Operation
    Cross out the Tractor Operation line.
  9. Certifying Authority
    The extension faculty or authorized agricultural education instructor who issues the certificate. This signature indicates that the youth is 14 years of age and has completed at training program.
  10. Person who conducted the training program
    Signature of one of the instructors involved with the training program.


Recordkeeping

The triplicate form indicates who shall receive each copy. It is recommended that the certifying authority is to make a photocopy of the "employer's copy" since this is the original and the carbon on the "certifying authority" copy make not be as clear over time.
